Not sure when the door was installed, I am guessing sometime in the past 15 years. No markings on anything to indicate manufacturer. The only thing I could find was on the inside handle the number 2806 is stamped and on the outside portion I found #8. Nothing else I could see about make or model of the handle or door.

The handle is about 7.5 inches long with the center-to-center measurement of the two screw holes being just shy of 4". Would like to get whole new assembly, or at the very least the outdoor portion, which is the one on the left below, that is slowly rotting away.

That's good thinking about replacing the hardware before it fails & becomes an emergency!

You mentioned "about" and just shy of" for measurements. Could it be metric? This will help you marrow down the search.

Maybe A Match Here-
Hager Company makes a very wide variety. Perhaps they have hardware to replace yours. https://www.hagerco.com/sliding-door-hardware You have to find the item at Hager and order from a dealer.

OR-
Is there a locksmith or "Lock and Key" shop with a storefront near you?
They are a surprisingly good resource for identifying and supplying unusual and obsolete door and window parts- including handles.

Bring the parts and be sure to measure the distance between holes in the door exactly on-center & measure the holes' diameter. (Drill bits are good for measuring hole sizes, usually 0.002" under.)

Sometimes the holes in the door are slightly larger than the bosses on the removable part. This may allow a "close but not exact" handle that you like to fit.


OR-
Mom & Pop hardware stores are also good resources for door hardware. If they don't have the parts, perhaps they will order them.

It may (or may not) be worth checking at a chain hardware store just in case it's a popular part. Big box? Maybe maybe not.
